General
Can I register my team early Saturday morning?
No. All teams are required to use the registration process for the 2011 tournament. For 2011 tournament you can either mail in your registration items or come to the Friday night registration. The process is listed in a question below.
Can USYSA registered players participate as guest players on a US Club registered team? If so, is there any paperwork or fees that need to be processed?
US Club Soccer is the deciding entity in terms of if they will allow a USYSA player to guest on one of their registered teams. US Club Soccer indicates that yes, a USYSA registered player can participate as a guest player on a US Club registered team. The USYSA registered player, must follow the rules of his respective USYSA State Association, and the tournament in regards to guest players (player loans). As the guest player is registered through USYSA he must have all appropriate approvals and paperwork required by his respective State Association. US Club Soccer will not require any paperwork for a guest player who is carded with USYSA.
How many divisions per age group will the tournament have in 2010?
The Team Ohio F.C. Nike Challenge Cup anticipates that each age group will consist of an Elite and Premier & Select division. The Elite division will be the upper division. Each division will be targeted to contain 8 teams. Although some divisions may contain 10 or 12.
I am a US Club Soccer registered team and I am required to bring a "Letter of Good Standing" with me to registration. Where do I get that ?
US Club Soccer "Letters of Good Standing" may be obtained from US Club Soccer by (1) logging into their website as a member at http://www.usclubsoccer.org with your username and password, (2) select Registration Forms, and (3) download form C011 Membership Confirmation Memo (aka Letter of Good Standing). Submit the form with the remainder of your requirements at Team Registration.
I have misplaced my original registration forms. How can I get duplicates ?
Downloadable versions of the form in Word and/or PDF file format were made available to you when your team was accepted and you confirmed your participation. If you need a duplicate form and have already confirmed, you will need your TEAM ID Number provided to you to access the forms. In the tournament website, http://www.Ohio-FCNCC.com log in with your TEAM ID Number, and the system will tell you that your team has already been confirmed. There will be a link to the page that contains duplicate forms. We recommend the use of the Word file format.
If a player is 18 years old or older, does he still have to complete the Medical
Release Form and the Sports Ohio Liability Consent Form?

Yes, ALL players must present a current and notarized Medical Release Form, and a signature on the Sports Ohio Liability Consent Form. If a player is 18 or older, he is legally capable of signing as the responsible party and is permitted to do so.
We have medical release forms on file from our league play and/or recent tournament. Can we use these instead of the forms from the Web site?
Yes, you may use your medical release forms, but they must be forms filled out from this season (2009/2010), i.e., you cannot use a form that was filled out for last season (2008/2009).
WHAT DO I NEED TO BRING TO REGISTRATION?

USYSA REGISTRATION REQUIREMENTS
If registering under USYSA you will be required to provide:

1. Original State Approved Roster (plus two copies which will be retained)
2. Player Pass Cards (for each player and guest player)
3. Coach Pass Cards (for each registered coach)
4. Notarized Medical Release Forms (for each player) - These forms can be from your own organization or State Association. You do not need to present these. Just have them on hand.
5. Sports Ohio Liability Release Form (must be signed by ALL players & parents) If you are scheduled to play at Soccer First.
6. Travel Permit (if from outside of Ohio South)
7. Guest Player Roster (if applicable)
8. Registration Checklist

US CLUB SOCCER REQUIREMENTS
If registering under US Club Soccer affiliation, you will be required to provide:

1. Approved Official Roster (from your member organization)
2. Letter of Good Standing (from your organization)
3. Notarized Medical Release Forms (for each player) � - These forms can be from your own organization or area USYSA State Association but they must be notarized. You do not need to present them. Just have them on hand.
4. Player Passes (for each player) � Passes MUST BE LAMINATED
5. Coach Passes (for each coach) � Passes MUST BE LAMINATED
6. Sports Ohio Liability Release Form (must be signed by ALL players & parents)

Teams may have no more than 18 players for 11v11 play (age groups U12-U19). Teams may have no more than 14 players for 8v8 play (age groups U11 and U12). Teams may have no more than 12 players for 6v6 play (age groups U10 and younger). Four guest players per team will be permitted as long as they are currently USYSA registered and possess a stamped player pass card in the correct age group.

For 2012, we'll offer two options: Mail-in/Email registration or Friday night in-person registration.

• All rosters sent in via mail-in registration will be frozen as of April 6, 2012 for girls and April 13, 2012 for boys.
• All teams and players will determine their ages per the USSF/USYSA age guidelines. For example, all players born on or after August 1st, 1997 would be U-13.
• A player may not be rostered to more than one team.

Documentation Requirements

**All forms requested by the Club Ohio Nike Challenge Cup will be available for download upon team acceptance.

USYS Teams:
• One copy of the official stamped and validated roster to leave with tournament. (Roster frozen once sent) 
Total roster sizes are up to a maximum of eighteen (18) players for teams playing 11v11, a maximum of fourteen (14) players for teams playing 8v8, twelve (12) players for teams playing 6v6. No player shall be allowed to register with more than one team or switch to another team during the tournament.
• A copy of the validated player cards, on both sides, rostered players and guest players. 
 For in person registration, just present the cards.
• A copy of the validated Coaches pass card on both sides, one for each, if applicable. Maximum of 3. 
 For in-person registration, just present the cards.
• Copy of Travel permit if your team is from outside of OSYSA. 

• Copy of Guest Player roster approved and validated by the state in which the player is registered. 

• Copy of the Soccer First Roster and Release Forms if playing at the Soccer First Facility
• Copy of both sides of the of the Guest player card, maximum of 4 guest players.

US Club Teams:

No player shall be allowed to register with more than one team or switch to another team during the tournament.
• A copy of both sides of the validated Player Pass. 
 For in-person registration - just present the passes.
• A copy of both sides of the validated Coaches Pass for each Coach, maximum of 3. For in-person registration - just present the passess.
• Copy of the Player Loan Form for all guest players. 

• Copy of both sides of the guest player cards-maximum of 4 players. For in-person registration - just present the passes.

• Must provide the documentation listed above plus:
• A completed form from their Provincial or National Association approving the team's request to participate in the event.


For mail-in registration, an email confirmation will be sent to the email provided on the Registration Checklist once your registration is processed.
Where is the Sports Ohio Liability Release?
When you confirmed after your acceptance from the Tournament, you were sent to a page that had ALL the registration forms and requirements outlined in detail. The form is still there. You should use your TEAM ID NUMBER to log in and make sure you have ALL the materials you will need for a successful registration.

Hotels/Travel
Our team needs a hotel. Can you help us with that?
The Tournament Committee has secured well over 3000 hotel rooms throughout central Ohio. To locate a hotel participating with the tournament just locate the link "Find a Hotel" on the front page. From there you can call or e-mail the participating hotel of you choice. All teams are responsible for arranging their own housing.

There are some important dates to be aware of when securing hotel rooms. Reservations not made by that date will be released & rebooked.

All cancellations made after 4/6 for the girls and 4/13 for the boys will result in the first nights room and tax being charged to the credit card attached to the reservation.

It is the committees hope that these new guidelines will assure that there will be a sufficient number of rooms available for all of our tournament guest.
What is a "stay and play" tournament?
Simply stated, if your team requires hotel accommodations and is accepted to this tournament, your team must stay at one of the participating tournament hotels. These participating hotels are reviewable on the "hotel" link on the front page area on the left hand side.
Why is this tournament a stay and play tournament? and what is the penalty?
We work very hard to keep our costs down and also balance the economic impact a tournament like ours can have to support the city in which Team Ohio FC and the Nike Cup are headquartered. The approved hotels on this website assist with both concerns. To make it managable we have 3000 room nights "blocked" for our out of town teams from these combined hotels. You'll find price and amenities of all ranges.

An accepted team to the 2012 tournament who requirea overnight accommodations and does not book with our approved hotels will forfeit their accepted status. The Nike Cup does not use a Hotel Service Bureau.

Web Site
The application form does not look right on my screen (overlapping drop down boxes, long rows over the input fields, etc.) What is going on?
First, you should let the ENTIRE form load BEFORE you scroll. It is a big form.

In short, your computer does not have enough memory (RAM) available to render the form correctly. Here is the solution that will work in most cases:

1. Clear the cache (temporary files) in Internet Explorer (Usually Tools>Internet Options>Remove Temporary files
2. Shut down Internet Explorer
3. Restart the computer
4. Open JUST MS Internet Explorer.
5. Go to Web site, fill out form.

In most cases, this will solve the problem....
